Videos Consent

I hereby grant the West Nova Chamber of Commerce (WNCC), its directors, officers, employees, agents, and designees (collectively referred to as “WNCC”) irrevocable permission to capture my image and likeness in videotapes, motion pictures, recordings, or any other media (referred to as “Images”). I acknowledge that WNCC will own these Images and further grant WNCC permission to copyright, display, publish, distribute, use, modify, print, and reprint these Images in any manner related to XYZ COMPANY business. This includes, but is not limited to, their use in publications, advertisements, brochures, web site images, or other electronic displays and transmissions thereof. I waive any right to inspect or approve the use of the Images by WNCC before their use. I release and hold WNCC harmless from any and all liability arising from the use of the Images in any manner or media and waive any and all claims and causes of action related to the use of the Images. This includes, but is not limited to, claims for invasion of privacy rights or publicity. I hereby warrant that I am eighteen (18) years of age or older and competent to enter into this contract in my own name. If I am less than eighteen years old, I confirm that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assignees, and personal representatives. I shall retain the rights to link to video’s posted on the WNCC web site.
